Community primary health care focuses on providing accessible, comprehensive, and equitable health services to individuals and families within the community. It emphasizes prevention, health promotion, and the management of common health issues, fostering collaboration between healthcare providers and community members to address local health needs effectively.

Health Promotion and Injury/Illness Prevention
Health Promotion and Injury/Illness Prevention initiatives encompass various strategies to enhance community well-being. Through health education, individuals gain vital knowledge about healthy practices and disease prevention. Additionally, First Aid at Community Events, such as Pow-wows and Culture and Language Camps, ensures prompt medical assistance, safeguarding participants’ safety and preserving cultural traditions. These efforts collectively empower communities to prioritize health and safety, fostering resilience and vitality.

Maternal Child Health

Maternal Child Health programs offer comprehensive support throughout pregnancy and early childhood. They include prenatal/postnatal assessments, education, and referrals. Additionally, prenatal classes and workshops provide essential guidance, complemented by traditional parenting resources. Educational initiative ensure child well-being. Additional services promote maternal and child nutrition and oral health, fostering healthy futures for families.

Immunization

Our Childhood Immunization program starts at 2+ months, followed by School-age vaccinations and health education. Flu & COVID-19 clinics begin at 6+ months. Adults receive vaccinations like Pneumo-23 and Tetanus, with home visits available. This program is essential to managing the risk associated with communicable disease now and in the future.

Communicable Disease Control

Our clinic provides STBBI Testing and Treatment, including Chlamydia/Gonorrhea (urine), Syphilis/Hep C/HIV (blood test), Tuberculosis testing. We offer support for dog bites, ensuring education and follow-up care. Additionally, we prioritize treatment, contact tracing, and follow-up for all communicable diseases. We distribute COVID-19 Rapid Self-Tests and offer home visits for personalized assistance.

Harm Reduction/ Exchange Program

Our Harm Reduction/Exchange Program provides supplies to mitigate substance use harms, offers education, and facilitates referrals to Mental Health, Addictions, and STBBI testing services. We distribute Narcan and offer hygiene incentives for returns. This is a shame-free approach designed to address addiction in our community, and keep members safe from the risk associated with substance abuse.

Drinking Water

Our Drinking Water Program conducts water testing, facilitates reporting, and offers home visits to ensure the safety and quality of drinking water in our community.
