Medical Transportation
Our main objective is to get the clients to their medical appointments on time.
TYPES OF TRANSPORTATION
- Medical taxi service to and from appointments, requires 24-hour notice.
- Private mileage, available is specific circumstances
- Transportation for out of province specialist appointments, 72-hour notice required
- Transportation to traditional healers
The transportation services we provide ensure the timely arrival and pick up of patients to medical appointments. Through this department Willow Cree Health Services works collaboratively with doctors, government, Indigenous communities, and non Indigenous stakeholders to deliver patients where they need to go.
THE REQUIREMENTS
Medical Taxi
24 HOURS NOTICE PRIOR TO TRANSPORTATION
The Medical Transportation Coordinator must be provided with the following client information:
- Name of Client
- Client’s Status number
- Date of birth
- Doctor’s name and specialty
- Date of appointment
- Time of appointment
- Location of pick up
- Phone Number (if any)
- Escort name (if client is under 18 or the client is medically incapacitated, or if client is going for surgery)
Private Mileage
We have assisted numerous clients with private mileage to get to their appointments, especially on holidays or when the taxis were not running and some with special circumstances.
24 HOURS NOTICE PRIOR TO TRANSPORTATION
The Medical Transportation Coordinator must be provided with the following client information:
- Clients must have a signed Certificate of Medical Attendance (CMA)
- Name of Client
- Client’s Status number
- Date of birth
- Doctor’s name and specialty
- Date of appointment
- Time of appointment
- Location of pick up
- Phone Number (if any)
- Escort name (if client is under 18 or the client is medically incapacitated, or if client is going for surgery)
Transportation Out of Province
72 HOURS NOTICE PRIOR TO TRAVEL
For specialist appointments, surgeries or for appointments that require you to travel out of province The Health Canada Call Centre requires prior approval for requests for private mileage, meals and accommodations and sometimes in-city taxi for out of province travel.
The Medical Transportation Coordinator must be provided with the following client information:
- Clients must have a signed Certificate of Medical Attendance (CMA)
- Name of Client
- Client’s status number
- Date of birth
- Doctor’s name and specialty
- Date of appointment
- Time of appointment
- Location of pick up
- Phone Number (if any)
If client is going for surgery, is medically incapacitated, or If client is under 18:
- Escort name
- Escorts status number
- Escorts date of birth
Transportation to Traditional Healer
72 HOUR NOTICE PRIOR TO TRAVEL
The Medical Transportation Coordinator must be provided with the following client information:
- A letter from their doctor indicating that they acknowledge the client’s request and that the client is in good medical condition to travel
- A letter from leadership indicating that the healer is recognized as such in the destination community
- Name of Client
- Client’s status number
- Date of birth
- Doctor’s name and specialty
- Date of appointment
- Time of appointment
- Location of pick up
- Phone Number (if any)
Reminder:
- Private mileage is only paid to the Saskatchewan border
- Any offerings, gifts, cloth, and tobacco are the responsibility of the client
